Question:
My question has to do with the diagnosis of my nephew and namesake (he turns 12 next month). Just today he was diagnosed with Crohn's by a pediatric gastro specialist in New Orleans. I would like to encourage my sister and brother-in-law to get a second opinion, just to be sure. So, here are the questions: Is the diagnosis so straightforward that there's usually no need for a second opinion? And if no, who're the top pediatric Crohn's experts in the USA? The world? (Actually, I'd like to know the answers to the last questions more that the first).
Answer:
I don't know where you live, but there are 2 docs at u of m med cntr in Ann arbor referred to me, and I already went there. Email me and I will give you the names. I will know more after my colonoscopy, but my internal med doc told me their internal medicine dept is the best in the country. My allergist concurred. I am going there, I take a wait and see attitude on these things (been down the recommended road, and it is invariably stickly a matter of opinion in most of my cases). I hope this is true. i have a problem.....is autoimmunity...probably not Crohn’s but is an issue related to the gut....so therefore the scope and who knows.... If they did a colonoscopy and biopsy and saw the Crohn's it is definite. Crohn's has a specific look as described to me a cobblestone look. What tests did they do to come up with the iagnosis? What symptoms is he displaying? As for the Doctor, I don't have a name but one of the best pediatric hospitals is The University of Miami Pediatric Center In Miami, Fl. I am sorry to hear about your nephew. The dx of CD is not easy and often takes an extremely long period of time. Seeking a second opinion is not a bad idea. Make sure you seek out a top Pediatric GI, preferable one who specializes in IBD. The best place to look is at a teaching hospital. The other thing to do is learn, learn, learn. I, too, have CD and my web site gives lots of information and links; http://www.skyweb.net/~mgbio/cd/cd.html. I don't know if the diagnosis of Crohn’s disease is that clear cut. If you or the family have any question about your nephew's diagnosis, a second opinion is always in order. Second question. The Mayo Clinic has several pediatric clinics which have good reputations. Last year one of our cousins took their 3 yr old son to one of them for a second opinion. The dr at the Mayo Clinic continues to work with their local dr, with treatment advice. Their son is doing well now. He had, I believe, sluggish colon. This was the same diagnosis their local dr came up with. The Mayo Clinic doctor's advice was very helpful for their local dr to get the problem treated successfully. (I don't know much in the way of details, but I do know their son is doing well. I saw him just a couple weeks ago while we were on vacation). I think you should check out Vanderbilt. There's a Dr.Peach there. A friend had a baby born with no colon or anus, but he's fine now under Dr.Peach's care. It's in the south- Nashville, TN.
